Welcome to the YPN® Walmart Prescription Savings Program. 

This form entitles you to special negotiated pricing on prescription drugs at

any Walmart, Sam’s Club or Walmart Neighborhood Market pharmacy.

On your next visit to one of the Walmart pharmacies, simply present this form. The pharmacy staff will then obtain the appropriate information from you. Keep this form with you. Should you use more than one of other Walmart pharmacy locations, you will need to present this form at each location the first time you use it.

This program is for patients and their families who are un-insured and cannot be used with any other insurance, including government insurance.

It’s easy to transfer your prescriptions and take advantage of these savings:

1. Call your local Walmart, Sam’s Club or Walmart Neighborhood Market pharmacy and provide the information located on your prescription containers.

2. Visit your local Walmart, Sam’s Club or Walmart Neighborhood Market pharmacy and bring your prescription container(s). The pharmacist will take care of the rest.

3. Click to www.walmart.com, click on “Pharmacy”, and then click on “Transfer.” (If you do not currently have an account on Walmart.com, you will need to register first.)

Who is entitled to use the YPN Prescription Form?
As a recipient of this form, you and any member of your household may use the prescription benefit.

What is the value of this program?
As a recipient of this form you have access to preferred pricing each time you fill a prescription at a Walmart Pharmacy.

How do I check the price for my prescriptions?
For an estimated price on what a medication will cost, call 1-800-972-0943, 8 am to 5pm CST. Identify yourself as a YPN Rx Program member and have the name of the drug, strength and quantity.

What drugs are available for $4?

1. •Your prescription may be available for just $4 for a typical 30-day supply or $10 for a 90-day supply.
   

2. •The $4 Prescription Program has saved customers more than $3 billion at our pharmacies.
   

3. •Walmart pharmacies also fill prescriptions for approximately 300 generic medications at $10 for a 90- day supply, saving customers time and additional money. The offer is available in-store.
   

4. •Walmart and Neighborhood Market Pharmacies offer a $4 Over-The-Counter (OTC) Program bringing customers more than 1,000 OTC items priced at $4 or less.
